Randomized Controlled Trial

Circulating nitric oxide pathway metabolites in heart failure with preserved ejection fraction: a sex-stratified cross-sectional analysis.

Sex Differences in NO Metabolites Are Limited and Metabolite-Specific in HFpEF — In a sex-stratified analysis of 171 HFpEF patients, circulating symmetric dimethylarginine (SDMA) was consistently higher in men than in women, with a small absolute difference of approximately 0.11 µmol/L (β −0.11 µmol/L, 95% CI −0.20 to −0.03), while the broader nitric oxide metabolite profile showed substantial overlap between sexes.

Effect of an Advanced Community Paramedic Program to Shorten or Prevent Hospitalizations: A Pragmatic, Point-of-Care, Randomized Clinical Trial.

Community Paramedic Home Program Fails to Reduce 30-Day Acute Care Use in Complex Patients — A pragmatic randomized trial found no meaningful difference in days alive outside hospital or emergency department between patients receiving a mobile community paramedic home program and those receiving usual care (26.7 vs 27.9 days out of hospital within 30 days).

Promoting shared decision-making in colorectal cancer screening in primary care: A cluster randomized controlled trial.

Mailed SDM Intervention Increases FOBT Prescribing, But Selection Bias Clouds Results — A mailed shared decision-making intervention significantly increased the proportion of Swiss primary care physicians who had at least one patient tested or planning fecal occult blood testing compared to controls — about 52% more likely in the intervention group (84% vs. 56%; RR 1.52, 95% CI 1.13 to 2.04).

Mezigdomide, carfilzomib, and dexamethasone versus carfilzomib and dexamethasone in patients with relapsed or refractory multiple myeloma (SUCCESSOR-2): a phase 3, open-label, randomised controlled trial.

Mezigdomide Triplet Nearly Doubles Progression-Free Survival in Refractory Multiple Myeloma — Adding mezigdomide to carfilzomib and dexamethasone more than doubled median progression-free survival compared with carfilzomib and dexamethasone alone — 18.0 months versus 8.3 months — representing approximately a 52% reduction in the risk of progression or death (HR 0.48, 95% CI 0.36–0.63; p<0.0001).

Results of the Pediatric Guideline Adherence and Outcomes (PEGASUS) Program Versus Usual Care Pragmatic Randomized Controlled Trial in Severe Traumatic Brain Injury.

PEGASUS TBI Guideline Program Fails to Outperform Usual Care Overall, But Improves Adherence in Isolated Injury — A multicenter pragmatic randomized trial found that the PEGASUS facilitated guideline implementation program did not improve overall ICU TBI guideline adherence compared to usual care in children with severe traumatic brain injury, with an adjusted difference of virtually zero (0.1%, 95% CI −3.3% to 3.5%).

Intravascular Ultrasound-Guided or Angiography-Guided Complex High-Risk PCI.

Routine IVUS Guidance Fails to Reduce Target-Vessel Failure in Complex High-Risk PCI — In a randomized trial of 2,020 patients undergoing complex high-risk percutaneous coronary intervention, routine IVUS-guided PCI did not reduce the rate of target-vessel failure compared with angiography-guided PCI alone — with events occurring in 13.9% versus 11.1% of patients, respectively, representing a numerically higher but statistically non-significant risk in the IVUS group (HR 1.25, 95% CI 0.97–1.60; P = 0.08).

Oral Levosimendan for the Treatment of Pulmonary Hypertension due to Heart Failure With Preserved Ejection Fraction: Rationale and Design of the LEVEL Trial.

LEVEL Trial Launches First Phase-3 Test of Oral Levosimendan in PH-HFpEF — The LEVEL trial is a phase-3, randomized, placebo-controlled study enrolling 230 patients with pulmonary hypertension due to heart failure with preserved ejection fraction (PH-HFpEF), designed to determine whether oral levosimendan improves exercise capacity over 12 weeks — no efficacy results are yet available, as this publication reports rationale and design only.

Effects of a combination of olive leaf extract and potassium on blood pressure in participants with mild to moderate hypertension: A double blind, randomized, placebo-controlled trial.

OLE-Potassium Supplement Reduces Morning Systolic BP and Improves Lipid Profile in Mild Hypertension — In adults with untreated mild to moderate hypertension, 12 weeks of daily olive leaf extract combined with potassium reduced morning systolic blood pressure by 5.4 mmHg on home monitoring, alongside reductions in total cholesterol by 11.1 mg/dL, LDL-cholesterol by 6.9 mg/dL, triglycerides by approximately 22 mg/dL, and oxidized LDL by roughly 23% compared to placebo.

Efficacy and safety of Gepotidacin for urogenital infections: a systematic review and meta-analysis of randomized controlled trials.

Gepotidacin Matches Standard Antibiotics for Urogenital Infections but Raises Safety Concerns — Across three randomized controlled trials enrolling 3,756 patients, gepotidacin achieved microbiological cure rates for urogenital Neisseria gonorrhoeae statistically comparable to those of standard antibiotics, with no meaningful difference in efficacy (RR 1.10, 95% CI 0.96–1.27; p = 0.17).

Efficacy of intermittent fasting on blood glucose and weight in type 2 diabetes and prediabetes: A comparison with ad libitum and continuous energy restriction diets.

Intermittent Fasting Outperforms Ad Libitum Diet and Matches Continuous Energy Restriction for Glycemic Control in T2DM — In a meta-analysis of 14 randomized controlled trials involving 899 patients with type 2 diabetes or prediabetes, intermittent fasting produced meaningfully greater reductions in HbA1c compared to an unrestricted diet (moderate effect size: SMD −0.64, 95% CI −1.04 to −0.24), while achieving outcomes statistically comparable to continuous energy restriction diets across all metabolic markers.
